We are seeking a board-certified MD or DO to serve as Chief Medical Officer at a 47-bed skilled nursing facility in beautiful Northwest Montana. This is a high-impact 1099 Locum Tenens contract position providing essential medical leadership and care to a stable resident population (average census ~30).
This role offers significant schedule flexibility once established, including a hybrid remote/on-site option. We are looking for a committed physician for a minimum 6-month contract (ideally 6–12 months) to ensure continuity of care for this deserving community.

Position Highlights
- Compensation: $200.00 per hour regular rate | $250.00 per hour callback rate
- Total Earnings Potential: $190,000+ in just 6 months
- Schedule: Monday–Friday, 8:00 AM – 5:00 PM
- Weekday call only (no weekend call required)
- Hybrid remote + on-site flexibility available once established
- Contract Length: Minimum 6 months, up to 1 year (ASAP start)
Key Responsibilities
- Provide medical direction and oversight for a 47-bed skilled nursing facility
- Perform new admission History & Physicals
- Complete 60- and 90-day resident assessments
- Conduct medication reviews
- Manage acute conditions (e.g., UTIs, falls, infections)
- Perform daily rounding
- Handle weekday call for urgent resident issues
Requirements
- Active Montana medical license (MD or DO)
- Board Certification in a primary care specialty (Family Medicine, Internal Medicine, or Geriatrics preferred)
- Active BLS certification
- Active DEA registration
- Strong background in primary care and urgent care
- Experience in long-term care / skilled nursing is highly preferred
