Job Title: Nurse Case Manager II
Location: Richmond, VA
Duration: 03 Months
Pay rate: $40.00 - $44.00 on W2
Job Description:
Key Responsibilities:
- Conduct comprehensive assessments using clinical tools and data review
- Evaluate member eligibility, benefits, and healthcare needs
- Develop and implement case management plans
- Coordinate with internal teams and external providers/services
- Ensure compliance with regulatory, accreditation, and company guidelines
- Promote cost-effective and quality healthcare outcomes
Required Qualifications:
- Active, unrestricted RN license (VA State)
- Minimum 3 years of clinical experience (hospital, home health, or ambulatory care)
- Experience in healthcare and/or managed care industry
- Strong computer skills (ability to navigate multiple systems)
- Excellent verbal and written communication skills
Preferred: Case Management experience & CCM certification
