Requirements

• Active RN License
• Minimum 3 years of Long Term Care Nursing Experience
• BLS Certification Required
• Strong assessment and documentation skills
• Experience with skilled nursing and long-term care residents

Responsibilities

• Perform resident assessments and ongoing evaluations.
• Administer medications and treatments as ordered.
• Coordinate care with physicians and advanced practice providers.
• Assist with admissions and discharges.
• Provide wound care and monitor treatment effectiveness.
• Manage Gastrostomy Tubes, Urinary Catheters, CPAP, Tracheostomy Care, and Nebuliser Treatments.
• Assist residents with activities of daily living as needed.
• Maintain accurate documentation within Epic EMR.
