We are seeking a dedicated and skilled Physical Therapist to join our team. The ideal candidate will have a strong background in various physical therapy settings, including home health, outpatient, and inpatient care. This role involves developing and implementing individualized care plans to help patients regain mobility and improve their quality of life. The Physical Therapist will work closely with patients recovering from orthopedic surgeries, injuries, or chronic conditions, ensuring they receive the highest standard of care.

Duties

  • Conduct comprehensive assessments of patients' physical abilities and limitations.
  • Develop personalized treatment plans based on patient evaluations and goals.
  • Implement therapeutic exercises and techniques, including trigger point therapy.
  • Educate patients and their families on rehabilitation processes and home exercise programs.
  • Collaborate with healthcare professionals to coordinate care for patients.
  • Monitor patient progress and adjust treatment plans as necessary.
  • Participate in discharge planning to ensure continuity of care after leaving the facility.
  • Maintain accurate documentation of patient treatments and progress notes.
Requirements

  • Degree in Physical Therapy from an accredited institution.
  • Valid state licensure to practice as a Physical Therapist.
  • Knowledge of anatomy and physiology relevant to physical therapy practices.
  • Experience in home health, outpatient, or inpatient settings preferred.
  • Familiarity with orthopedic rehabilitation techniques is a plus.
  • Strong communication skills for effective interaction with patients and healthcare teams.
  • Ability to develop comprehensive care plans tailored to individual patient needs.
