Job Summary\n

Cuready Healthcare Staffing is seeking a qualified RN Case Manager to support patient care and clinical operations within healthcare facilities. The ideal candidate will deliver safe, high-quality care while collaborating with interdisciplinary care teams and maintaining compliance with facility policies and professional standards.

Responsibilities\n

\n
• Coordinate care and discharge planning for patients with complex medical needs.
\n
• Participate in family meetings and collaborate on treatment plans.
\n
• Organize services such as DME, SNF, and ambulance transport independently.
\n
• Maintain compliance with CMS and The Joint Commission standards.
\n
• Perform needs assessments and coordinate community resources, especially for vulnerable populations.
\n
• Float to the Laguna Facility as needed and manage a patient ratio of 1:25.
\n

Qualifications\n

\n
• Active RN license in California, or pending with the ability to obtain prior to start date.
\n
• Minimum of 2 years acute medical Care Manager experience.
\n
• Proficient in Epic charting system.
\n
• Experience in acute hospital settings and care coordination.
\n
• Strong knowledge of HIPAA guidelines and OSHA regulations.
\n
• Prior travel experience required; first-timers not accepted.
\n
