Primary Care Physician (Family Medicine / Internal Medicine) - Lewes, DE

Job Type: Full-Time (Perm)

Setting: Outpatient Private Practice

Schedule: Mon - Thurs (8am - 4:40pm)

• Friday: Half-day (Telehealth: 8am - 12pm)

• No weekends or on-call requirements

Position Overview:

We are seeking a dedicated and compassionate Primary Care Physician to join a modern, technology-driven outpatient practice. This role focuses on preventive care, chronic disease management, and coordinated patient care within a collaborative, multi-specialty environment.

Key Responsibilities:

• Provide comprehensive primary care services to adult patients

• Diagnose and manage acute and chronic medical conditions

• Collaborate with specialists and care teams for coordinated treatment plans

• Manage a patient panel with an average of ~16 patients per day

• Participate in telehealth services (half-day Fridays)

Qualifications:

• MD or DO from an accredited medical school

• Board Certification in Family Medicine or Internal Medicine (or equivalent)

• Active or eligible Delaware medical license

• DEA registration

• Open To: New graduates (no minimum experience required)

Performance-Based Incentives:

• 20% bonus on ACO revenue

• 50% share of Chronic Care Management (CCM) fees

• 50% revenue share after 72 patients/week threshold
