Position Summary:

We are seeking a compassionate and patient-focused Primary Care Physician (PCP) to join a organization serving the senior population. This role operates within a collaborative, team-based care model designed to improve patient outcomes through coordinated care, longer patient visits, multidisciplinary support, and a concierge-style healthcare experience.

The PCP will work closely with advanced practice providers, pharmacists, behavioral health specialists, referral coordinators, coders, and medical assistants to provide comprehensive, high-quality care for geriatric patients.

Responsibilities:

• Patient Care & Clinical Excellence

• Evaluate and treat patients in accordance with established standards of care

• Provide comprehensive primary care services for geriatric/senior patients

• Manage patient panels of 500 or fewer patients

• See approximately 8-13 patients per day

• Participate in daily team huddles and collaborative care planning

• Ensure an exceptional patient experience and continuity of care

• Complete accurate and timely EMR documentation daily

• Collaborate with quality-based coders to optimize documentation

• Review quality outcomes and medical records with leadership

Qualifications:

• MD or DO from an accredited institution

• Active and unrestricted Alabama medical license

• Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine

• Active DEA license

• Medicare and Medicaid provider numbers
