Setting: Outpatient clinic
Schedule: Monday-Friday
Position Summary:
We are seeking a compassionate and patient-focused Primary Care Physician (PCP) to join a value-based care organization serving the senior population. This role operates within a collaborative, team-based care model designed to improve patient outcomes through coordinated care, longer patient visits, multidisciplinary support, and a concierge-style healthcare experience.
The PCP will work closely with advanced practice providers, pharmacists, behavioral health specialists, referral coordinators, coders, and medical assistants to provide comprehensive, high-quality care for geriatric patients.
Responsibilities:
Patient Care & Clinical Excellence
Evaluate and treat patients in accordance with established standards of care
Provide comprehensive primary care services for geriatric/senior patients
Manage patient panels of 500 or fewer patients
See approximately 8-13 patients per day
Coordinate specialty referrals, hospital care, SNF coordination, DME, and home health services
Participate in daily team huddles and collaborative care planning
Ensure an exceptional patient experience and continuity of care
Documentation & Compliance
Complete accurate and timely EMR documentation daily
Collaborate with quality-based coders to optimize documentation
Follow clinical protocols and leadership guidelines
Review quality outcomes and medical records with leadership
Team-Based Care Collaboration
Work closely with multidisciplinary care teams
Support a cooperative and professional clinic environment
Participate in quality improvement initiatives and value-based care programs
Qualifications:
Required Qualifications
MD or DO from an accredited institution
Active and unrestricted Alabama medical license
Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
Active DEA license
Medicare and Medicaid provider numbers
Strong interpersonal and communication skills
Experience or willingness to adapt to a value-based care model
Basic computer and EMR proficiency
TB screening required for patient-facing role
