Dates: starting ASAP and ongoing, 60-90 day credentialing timeline

Facility Details/Requirements:

• NBCRNA
• Indiana license required (we can help license you)
• Monday-Friday 7a–3p or 7a–5p
• Weekday call 5p–7a
• 24 hour weekend call with post-call day off
•  MDs onsite (no supervision)
• Case types: Cases: General, Endo, Ortho, Urology, Ophthalmology, OB/GYN, ENT, Radiology, Epidurals, spinals, blocks required (with logs)
• EMR is EPIC
