Trinity Rehab Services is hiring a PRN Physical Therapist to work across Skilled Nursing and Outpatient services in the Brook Park, OH area. You’ll join a collaborative, person-centered team with strong onsite support and realistic expectations.

What You’ll Do:

• Provide PT services under established plans of care
• Treat patients in SNF and/or Outpatient settings as appropriate
• Collaborate closely with an interdisciplinary team
• Deliver individualized, meaningful patient care
• Complete documentation efficiently and accurately

What We’re Looking For:

• OH Licensed Physical Therapist (PT) or Pending licensure
• New graduates encouraged to apply
• Communication and teamwork skills
• Commitment to high-quality, patient-centered care
