Job Summary
We are seeking a dedicated and reliable full-time Licensed Practical Nurse (LPN) or equivalent to manage medication flow charts and oversee medication reconciliation for our veterans. The LPN will be responsible for all aspects of medication management, ensuring safe and accurate medication administration and documentation. The role includes collaboration with a multidisciplinary team, who may assist in dispensing medications and administering toxicology screening, etc. This position requires strong attention to detail and commitment to providing high-quality care to veterans, their information, and teamwork.

Hours & Schedule: 5 days a week, 36-40 hours a week 

Key Responsibilities:

  • Manage and monitor medication charts, ensuring that all medications are accurately documented and administered in accordance with prescribed instructions.
  • Oversee medication reconciliation processes, ensuring that medications are appropriately adjusted, tracked, and recorded when there are changes in prescriptions or treatment plans.
  • Administer toxicology screening and records.
  • Coordinate with physicians, pharmacists, and other healthcare professionals to ensure accurate and effective medication management for each veteran.
  • Ensure compliance with all regulatory and organizational guidelines related to medication administration and safety.
  • Provide training and supervision to program staff on proper medication dispensing protocols and client care.
  • Document all medication related activities in the electronic health record system (EHR) and maintain accurate records for review and audits.
  • Provide direct care to veterans as needed, including administering medications, monitoring for side effects, and addressing any concerns related to medication adherence.
  • Serve as a point of contact for veterans and their families, providing education on medication regimens and answering any medication related questions.

Knoweldge & Skills

  • Strong understanding of medication management, medication reconciliation, and related clinical protocols.
  • Knowledge of medical terminology
  • Knowledge of laws, regulations, and best practices in medication administration.
  • Strong organizational and communication skills.
  • Proficiency in using electronic health records, electronic medical records or similar documentation software.
  • Ability to work collaboratively with other team members.
  • Compassionate, patient centered care with a focus on quality and safety.
  • Ability to manage multiple tasks and responsibilities in a fast-paced environment.
  • Excellent attention to detail, critical thinking, and problem-solving skills.
  • BLS (Basic Life Support) certification or higher

Experience

  • Valid licensure as a Licensed Practical Nurse (LPN) in accordance with state regulations.
  • Education & Licensing: Current, valid LPN license (or equivalent).
  • Proven experience working in diverse healthcare environments including hospitals, nursing homes, hospice care, or outpatient clinics.
  • Demonstrated proficiency with EMR/EHR systems.
  • Hands-on experience, techniques, and specimen collection/processing.
  • Knowledge of anatomy and physiology principles combined with strong patient assessment skills to deliver safe and effective nursing interventions.
  • Ability to work effectively with diverse populations, including seniors requiring geriatrics or memory care services, individuals with disabilities, or developmental challenges.
