Case Manager (Catonsville, MD)

Case Manager needed for our non profit client in Catonsville. The Case Manager will provide clinical and non-clinical care services for youth including on-going treatment planning and treatment interventions. Contract to hire position. The non-negotiable pay rate is $22.00 per hour.

Essential Duties:

• Establishes and maintains working relationship with community agencies in order to provide the most appropriate assistance to the children in placement.
  
• Develops and implements individualized treatment plans, providing both clinical and non-clinical interventions to youth.
  
• Establishes relationships with community agencies, schools, courts, and families to support placement stability, permanency, and resource connection.
  
• Conducts comprehensive assessments of youth and family needs.
  
• Performs child trafficking screenings, and creates safety plans.
  
• Ensures accurate, timely documentation in accordance with regulatory requirements and deadlines.
  
• Offers support for traumatic experiences, including support for medical or psychiatric appointments and crisis response.
  

Education:

• Must have at least two years of case management experience working with youth and families, preferably within a residential or foster care setting.
  
• A Bachelor's or Master's Degree in Social Work, or related field is required.
  
• Must be a licensed LMSW, LCSW-C, or LCPC.
  
• Must be able to work from 8:30 am-4:00 pm Monday through Friday with a 30 minute non paid lunch.
  
• Must be able to pass a FBI background check and drug testing.
