Case Manager – Bronx, NY
Pay: $25/hour Bilingual slightly higher.
Schedule: Full-Time | 40 hours/week
(Some evenings and weekends may be required)
We are seeking a dedicated and compassionate Case Manager to provide residential and long-term support services to homeless and at-risk adults transitioning into permanent housing in throughout the 5 boroughs. This position works closely with individuals experiencing housing instability and supports them in achieving long-term independence and stability.
The Case Manager will maintain a caseload of approximately 20–25 formerly homeless and/or low-income individuals and will provide ongoing support through face-to-face meetings, phone contact, outreach, and coordination with community providers.
Responsibilities
- Provide direct case management services both on-site and in the field
- Conduct mental health and substance use assessments
- Develop, implement, and update individualized service plans
- Assist clients with obtaining public assistance, Social Security benefits, and community resources
- Utilize evidence-based practices including Motivational Interviewing, Harm Reduction, and Critical Time Intervention
- Maintain accurate and timely documentation using electronic health records
- Coordinate clinical case management and psychosocial services
- Facilitate life skills, relapse prevention, educational, and support groups
- Complete referrals to medical, mental health, and outside service providers
- Conduct outreach, apartment visits, inspections, and engagement activities
- Work collaboratively with property management staff to support housing stability
- Mediate landlord-tenant issues when necessary
- Provide crisis intervention and trauma-informed support
- Participate in community meetings and community development initiatives
- Coordinate aftercare services and follow-up support post-discharge
- Ensure compliance with organizational and funding requirements
- Perform additional duties as assigned by leadership
Field work may account for up to 85% of the role in Scatter Site Programs.
Requirements
- Bachelor’s degree in Human Services or related field
- Strong organizational, communication, writing, and computer skills
- Experience working with low-income, homeless, or vulnerable populations preferred
- Fluent Spanish speakers strongly encouraged to apply
- Ability to work independently and within the community
- Must be comfortable conducting field visits and community outreach
Physical Requirements
- Frequent walking, standing, stooping, kneeling, typing, and reaching
- Ability to spend extended periods sitting at a desk
- Occasional lifting of items under 5 pounds
- Ability to move throughout facilities and the community during the workday
