RN Case Manager (OPWDD)

RN Case Manager needed to provide Case Management to the developmentally disabled within OPWDD group homes.

20-40 hours a week depending on your availability with some flexibility in regard to the hours and days a week.

Job Duties

• Updating and maintaining the individuals monthly MARS and health records as required by OPWDD.
• Schedule and follow up on the individuals medical appointments.
• Request MD orders or extensions on existing orders or extensions when necessary.
• Be available to triage individuals if anything should arise while you are in the group home.
• Body checks/Assessments when needed as well as following up with nursing notes.
• Supervise med passes as needed for AMAP Certifications.
• OPWDD experience preferred.

$50-$53 an hour
