Position Overview:
\nWe are seeking a dedicated Registered Nurse to provide skilled home health care across a variety of patient needs. This role focuses on delivering high-quality clinical services, including admissions and ongoing patient care management, while ensuring accurate documentation and collaboration with the care team. The ideal candidate is adaptable and comfortable working a flexible schedule that may include days, evenings, weekends, and on-call rotations.

Schedule & Work Details:

- Schedule: Flexible shifts including days, evenings, and weekends \n
- Weekly Guarantee: 36 hours \n
- Productivity Expectation: 30 points \n
- On-Call/Weekend Requirement: Every 3 to 4 weeks when fully staffed \n
- Mileage Reimbursement: Not provided \n
- Pay Rate: $51 to $53 per hour \n
Key Responsibilities:
\n- \n
- Perform Start of Care (SOC), Resumption of Care (ROC), and recertification visits \n
- Conduct comprehensive patient assessments and develop individualized plans of care \n
- Monitor patient conditions and update care plans as needed \n
- Provide skilled nursing interventions in accordance with physician orders \n
- Maintain accurate, timely, and compliant documentation \n
- Collaborate with physicians and interdisciplinary team members to ensure continuity of care \n
- Educate patients and caregivers on treatment plans, medications, and disease management \n
- Manage a dynamic schedule and caseload efficiently while meeting productivity expectations \n
Required Qualifications:

- Active Registered Nurse (RN) license \n
- Strong clinical assessment and documentation skills \n
- Ability to work independently and manage a flexible schedule \n
- Experience with SOC, ROC, and recertifications \n
- Preferred Qualifications: \n
- Prior home health experience \n
- Strong time management and organizational skills \n
- Excellent communication and interpersonal skills \n
- Ability to adapt to changing patient and scheduling needs \n
