Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to join our Home Health team. The RN will provide comprehensive, patient-centered care through clinical assessments, care coordination, patient education, and ongoing monitoring of patient conditions. This role works closely with physicians, therapists, social workers, caregivers, and other healthcare professionals to ensure high-quality care and positive patient outcomes in the home setting.
Key Responsibilities

• Conduct comprehensive assessments of patients’ physical, emotional, and overall health status.
• Develop, implement, and regularly update individualized care plans based on patient needs and physician recommendations.
• Educate patients and caregivers on chronic disease management, medications, potential side effects, and self-care techniques.
• Coordinate patient care with physicians, therapists, social workers, and other healthcare professionals.
• Monitor patient progress and promptly communicate changes in condition to the appropriate members of the care team.
• Maintain accurate and timely electronic medical record (EMR) documentation, including assessments, interventions, and progress notes.
• Supervise Structured Family Caregivers, Home Care Aides, and other caregiving staff to ensure quality care delivery.
• Ensure compliance with established care plans, agency policies, and healthcare regulations. Promote high-quality patient care, patient safety, and positive health outcomes.

Required Qualifications

• Active Registered Nurse (RN) license in the applicable state
• Associate Degree in Nursing (ADN or RN-to-BSN program)
• High School Diploma or equivalent
• Strong clinical assessment and patient education skills
• Knowledge of EMR documentation and care coordination processes
• Excellent communication, organizational, and interpersonal skills
• Ability to work independently and collaboratively within an interdisciplinary team

Preferred Qualifications

• Bachelor of Science in Nursing (BSN)
• Previous Home Health experience
• Previous Case Management experience
