Wesley by the Bay | Ocean City, NJ

Full Time Day Shift | No Weekends!
Monday - Friday working 8:30 am - 5 pm

Pay: $21.01-$23.63/hour
based on experience

The Housing Services Coordinator is an excellent opportunity for someone who enjoys working with older adults, brings creativity and initiative to their work, and is excited to help shape and grow a meaningful housing support program. In this role, you'll have the ability to make an impact and contribute fresh ideas while working within a well-established, mission-focused environment.

What You'll Do

As the Housing Services Coordinator, you will support residents in maintaining safe, stable, and connected living environments by linking them to essential services and programs. Key responsibilities include:

  • Connect residents and home-based participants with supportive services to promote successful aging in place
  • Assist residents and families with transitions to higher levels of care in coordination with external agencies
  • Develop and maintain a community resource guide for housing and supportive services
  • Provide regular outreach and follow-up for frail residents, including post-hospital and post-therapy check-ins
  • Track, document, and report service outcomes (monthly and annual reporting)
  • Create and distribute communication materials such as newsletters, website updates, and social media content
  • Serve as a liaison between residents, families, resident groups, and community partners
  • Plan and coordinate social, educational, spiritual, and vocational programs that reduce isolation and promote well-being
  • Maintain monthly activity calendars and support development of congregate and HCBS programs

What We're Looking For

Education (one of the following required):

  • High School Diploma or equivalent with relevant work experience
  • Some college coursework
  • Associate's degree

Experience:

  • 3+ years of experience in services for older adults, disadvantaged populations, or social services in a healthcare or related setting
  • 5+ years of experience and knowledge of entitlement programs, healthcare systems, and dementia care preferred
