About the Role:
Join VITAL HOME CARE AGENCY LLC as a Home Care Private Duty RN/LPN, where you will provide compassionate and personalized care to our young clients. We are dedicated to enhancing the quality of life for our clients while offering a supportive environment for our healthcare professionals. Immediate need in Randolph, New Jersey for 12 hour NIGHTS ( Tues and Thurs). Additional hours and shifts available.

Responsibilities:

• Administer medications and treatments as prescribed by physicians.
• Monitor and assess patient health status and report changes.
• Provide direct care and support to pediatric patients and young adults in their homes.
• Educate families on care plans and health management.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Maintain accurate and up-to-date patient records and documentation.
• Assist with daily living activities and personal hygiene.
• Ensure compliance with healthcare regulations and agency policies.

Requirements:

• Current RN or LPN license in New Jersey.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Compassionate demeanor with a focus on patient-centered care.
