The LPN provides personcentered nursing care, monitors chronic and behavioral health conditions, manages medications, coordinates medical appointments, and ensures compliance with state and agency regulations. This role plays a vital part in promoting safety, dignity, independence, and quality of life for individuals with intellectual/developmental disabilities and cooccurring behavioral or medical needs.
Key Responsibilities
Clinical Care & Monitoring
- Conduct daily nursing assessments and monitor vital signs.
- Identify and document changes in physical or behavioral health.
- Provide direct care for chronic conditions, acute medical needs, and behavioral health concerns.
- Respond to medical and behavioral emergencies using approved crisis prevention and deescalation techniques.
- Administer medications according to MARs, physician orders, and DBHDD standards.
- Update MARs, document medication changes, and ensure proper storage, labeling, and disposal.
- Communicate medication updates to staff and interdisciplinary team members.
- Schedule and coordinate all medical, dental, psychiatric, therapy, and specialty appointments.
- Ensure timely routine and followup visits based on provider recommendations and DBHDD requirements.
- Maintain an organized appointment calendar and arrange transportation as needed.
- Prepare individuals for appointments by gathering documentation and updating medication lists.
- Attend appointments when required, advocate for individuals, and relay provider instructions to the team.
- Document appointment outcomes, treatment changes, and followup needs in compliance with DBHDD standards.
- Track preventive care requirements such as annual physicals, labs, immunizations, and screenings.
- Complete all nursing documentation accurately and on time according to DBHDD, state, and agency guidelines.
- Maintain health records, incident reports, and care plans.
- Ensure compliance with DBHDD policies, HCBS waiver requirements, and individual healthcare plans.
- Participate in ISP meetings, nursing reviews, and quality assurance activities.
- Collaborate with physicians, pharmacies, behavioral health providers, and interdisciplinary team members.
- Train and support Direct Support Professionals (DSPs) on healthrelated tasks, infection control, and documentation standards.
- Promote a safe, clean, and therapeutic environment while supporting community integration and independence.
- Monitor for signs of abuse, neglect, or exploitation and follow mandatory reporting requirements.
- Advocate for individuals rights, preferences, and personcentered goals.
- Strong knowledge of DBHDD standards and group home regulations

- Compassionate, traumainformed approach
- Active Georgia LPN license in good standing
- Experience with IDD, behavioral health, or dualdiagnosis populations
- Knowledge of DBHDD policies (Preferred)
- Ability to pass background check
- Valid drivers license
- 1-3 years of experience working with people with mental disabilities and IDD
- Minimum of 1-2 years' experience with DBHDD policies, HRST processes
- Completion of an accredited practical nursing program
- BLS First Aid Certification
