Responsibilities:
Evaluate and develop personalized treatment plans
Provide high-quality physical therapy services to patients in their homes
Educate patients and families on treatment plans and progress
Collaborate with healthcare professionals to ensure excellent patient care

Qualifications:
Active Colorado Physical Therapist License
Strong communication and patient care skills
Home Health experience preferred, but not required
Compassionate, reliable, and patient-focused
