Job Description:
Seeking an experienced Registered Nurse – Case Manager to support inpatient care coordination, discharge planning, and interdisciplinary collaboration. The role focuses on safe transitions of care and coordination of post-acute services.
Responsibilities
Manage daily inpatient case management caseload
Participate in interdisciplinary rounds
Perform patient assessments and update care plans
Coordinate discharge planning and post-acute services including:
Home health
Inpatient rehab
Durable medical equipment (DME)
Nutrition services
Collaborate with physicians, care teams, and external providers
Document care coordination activities and place orders in Epic EMR
Requirements:
Active RN license
BSN required
Certified Care Manager (mandatory)
Minimum 3 years inpatient clinical experience
At least 1 year case management experience
Experience with Epic EMR preferred
