**Must have a Master’s Degree in Social Work**
Must be a Licensed Master Social Worker or have provisional license
Pay: Competitive pay, based on experience
Schedule: 8am-5pm Monday-Friday
Our Medical Social Workers are vital to our patient experience for those utilizing the hospice benefit. As a Hospice Medical Social Worker for Carter Healthcare, you will be leading our patients and their families through the navigation of end-of-life care. You will work closely with our interdisciplinary team to prioritize our patients’ psychosocial well-being through:
· Supporting clients with resources
· Providing education to patients and caregivers about coping skills, hospice philosophy, and non-pharmacological symptom management strategies
· Mediating conflicts within families, between clients and the interdisciplinary team, and between service organizations
· Advocating on behalf of the patient and family
· Identifying and linking patients with resources
· Documenting social work activities
You will spend much of your time in the home with our patients and their families facilitating advance care and lifespan planning. Hospice is a benefit that reaches far beyond our patients, and we consider it a privilege to offer these resources to our patients and their families in their time of need.

Qualifications:
·Master’s degree in social work from a school accredited by the Council on Social Work Education.
· A minimum of 2 years’ experience in home health or hospice preferred.
· Understands Hospice philosophy, and issues of death/dying.
· Experience in health care required.
