Clear Guidance is a care management and consulting practice out of Wellesley, MA. Our founder, Dr. Pilcher, has over 20 years experience working with elders and disabled adults. Our practice specializes in working with clients and their families who are challenged with cognitive impairment, young onset and atypical dementias and mental illnesses. We have an interdisciplinary team of a Gerontologists, social workers, nurses, occupational therapists, physical therapists and expressive arts therapists that work together to provide behavioral coaching, medical oversight and coordination and consulting on the challenges these families are facing.

Our practice is growing and we are looking to add to our team of specialists, so a passion and experience working with clients and families facing issues with dementia and mental health challenges is essential. We are willing to train care management skills to the right candidate who wants to transition to this field.
Duties and Tasks
- Conduct comprehensive in-home assessments and draft written summaries of findings to provide to clients and/or families within two days of assessment.
- Develop goal oriented written care plans within a week of the initial meeting with client, and provide to client and/or family.
- Implement various components of the care plan and update care plan on a regular basis, as appropriate
- Negotiate family dynamics to implement care plan. Work within existing family systems to ensure client's needs are met and families have the tools needed to communicate and work towards realistic goals
- Communicate regularly with family members and appropriate professionals
- Cultivate and maintain extensive knowledge of community resources available to older adults and those with disabilities
- Collaborate with professionals and advocate for clients’ needs
- Coordinate client’s medical providers and medications, collaborate with providers to ensure good communication and consistent disease management

Education and Experience
- Masters Degree in Social Work, Gerontology, Psychology, Nursing, or related field
- 5 years or more experience in the field of elder care or mental health setting
- Demonstrated clinical knowledge of aging, dementia, and/or disabilities
- Experience working with complicated family systems and family dynamics
- Knowledge of local eldercare resources (preferred)
- Experience working with mental health issues and coordinating services for clients with mental illness (preferred)
- An equivalent combination of education and experience will be considered
Expectations/Requirements

This position with begin as per diem with the ability to grow into a 32 hour benefitted position. We are especially interested in candidates who are willing to work in the Greater Boston area.
