\nThe Primary Care Physician (PCP) provides comprehensive, high-quality care within a collaborative, team-based environment focused on value-based outcomes.\n

\nPhysicians are supported by care teams and quality-based coders to ensure strong documentation, coordinated services, and efficient workflows — allowing providers to focus on meaningful patient interactions.\n
\n
\nThis is a structured model with minimal call requirements and clearly defined quality metrics tied to bonus compensation.\n
\n
\nKey Responsibilities\n
\n
\n• Deliver high-quality primary care aligned with clinical standards\n
\n• Coordinate comprehensive patient services, including:\n
\n– Specialist referrals\n
\n– Hospital and SNF coordination\n
\n– Durable medical equipment\n
\n– Home health services\n
\n• Participate in daily care team huddles\n
\n• Collaborate closely with Lead Physician and Center Administrator\n
\n• Complete daily medical record documentation\n
\n• Work alongside quality-based coders for accurate and comprehensive documentation\n
\n• Adhere to clinical leadership policies and protocols\n
\n• Review quality and outcome data regularly with leadership\n
\n• Focus on direct patient care and related administrative responsibilities\n
\n
\nRequired Qualifications\n
\n
\n• MD or DO from an accredited program\n
\n• Active and unrestricted medical license (or willingness to obtain in state of practice)\n
\n• Willingness to obtain regional state licensure as needed\n
\n• Board Certified or Board Eligible (Family Medicine, Internal Medicine, or Geriatrics preferred)\n
\n• Active and unrestricted DEA license\n
\n• Medicare and Medicaid provider numbers\n
\n• Strong interpersonal and communication skills\n
\n• Commitment to a team-based care model\n
\n• Willingness to adapt to a value-based care environment\n
\n• Basic computer and EMR proficiency\n
\n• TB screening required upon selection\n
\n

\n• HEDIS / STARS / CAHPS / HOS knowledge\n
\n• Medicare Advantage Global Risk Model experience\n
\n• Experience in medically underserved or culturally diverse communities\n
\n• Familiarity with electronic health records\n
