Overview
Are you a passionate Physical Therapist looking for a role where you can truly connect with your patients and enjoy a healthy work-life balance? We’re seeking a dedicated per diem Physical Therapist to join our team in a supportive, energetic, and patient-focused setting.
The Physical Therapist (PT) provides evaluation and therapy services, including direct patient care, patient/family education and transition of care, in an efficient, age-appropriate, and culturally sensitive manner upon licensed independent practitioner referral.
Compensation: The pay range for this position is $35.66 hourly to $53.49 hourly. Compensation is determined based on years of experience and department equity.
Responsibilities
- Upon receiving a licensed independent practitioner referral, implements a systematic patient examination and evaluation utilizing comprehensive compliant and safe clinical care demonstrating the philosophy of family centered care
- Works with medical staff and the multidisciplinary team to design and implement individualized plans of care to meet the needs of the patient and their family.
- Makes appropriate discharge plans by collaborating with patients, families and the interdisciplinary team.
- Functions as a team member and develops leadership skills through activities such as mentoring and providing education to students and other professionals.
- Supports risk management and compliance initiatives through accurate, thorough documentation.

Qualifications
Required:
- Education necessary to obtain License in State of Practice
- Minnesota PT license
- CPR/BLS (per hospital policy)
Preferred:
- 1 year physical therapy experience; will consider new grads
- Pediatric experience
