Job Summary:
The organization is seeking a Physical Therapist (PT) specializing in outpatient services in Lakeport, California. This position involves providing outpatient physical therapy care, with candidates required to bring their license on the first day. Job Details:
- Specialty: Physical Therapist Outpatient (PT OP)
- Location: Lakeport, California
- Schedule: 8-hour shifts, 5 days per week, with a minimum of 40 hours per week
- Required Qualifications: CA License, graduate of an accredited school of Physical Therapy approved by the American Physical Therapy Association with a B.S., B.A., M.A., M.S., or D.P.T. degree; 2 years of relevant outpatient experience; BLS certification.
- Preferred Qualifications: Epic electronic medical records experience.
- Shift: 8:00 AM to 5:00 PM, Monday through Friday.
- Additional Information: Candidates must be available to undergo lab work within 3-5 days of signing. All certifications must be from the American Heart Association or American Red Cross and be in hand at the time of submission. Applicants must include the Sutter Cover Sheet and two references obtained within the last year. A 7-year work history verification is required.
- Certifications & Documentation: COVID-19 and Flu vaccines are required. Certification copies must be current and in hand.
- Travel Requirements: 50 miles travel requirement; locals are accepted at the local rate, with acknowledgment that they are eligible for a lower billing rate.
- Other Requirements: Must comply with background policies; state if there are any criminal background hits or DUIs.
