Faithful Journey Home Care is seeking a dependable, professional, and client-focused Community Care Coordinator to join our team. This full-time position plays an important role in supporting clients, caregivers, families, and office operations while helping ensure quality care and strong communication across the agency.

Position Summary

The Community Care Coordinator is responsible for client and caregiver support, supervisory visits, care coordination, field relationship management, and assisting with daily operational needs. This role serves as a key point of contact for caregivers and backup for clients, helping resolve concerns, support care quality, and strengthen relationships between the office, caregivers, clients, and families.

Responsibilities

  • Complete client and caregiver supervisory visits
  • Communicate with caregivers through calls and texts
  • Provide caregiver support and in-person problem-solving
  • Assist with shift coverage and fill-in support as needed
  • Participate in activities, health fairs, and community events
  • Support scheduling needs while in the office
  • Participate in an every-other-week on-call rotation
  • Assist with intakes when assigned or when backup is needed
  • Support client and caregiver relationships
  • Help identify and resolve care coordination concerns
  • Communicate client and caregiver updates to leadership
  • Assist with call-ins, open shifts, urgent coverage needs, and field support
  • Escalate care concerns, complaints, safety issues, family concerns, compliance issues, or urgent matters as needed
  • Other duties as assigned

Qualifications

  • Experience in home care, healthcare, caregiving, or care coordination preferred
  • Strong communication and relationship-building skills
  • Ability to work professionally with clients, families, caregivers, and office staff
  • Strong problem-solving skills and ability to provide caregiver support in the field
  • Strong organizational and follow-up skills
  • Ability to participate in on-call rotation and rotating holidays
  • Ability to assist with health fairs and community events
  • Must be dependable, professional, and client-focused

Schedule

  • Full-time
  • Participation in daily team huddle
  • Every-other-week on-call rotation as assigned (Stipend Pay)
  • Rotating holidays as assigned by the agency

Compensation

Compensation is based on offer and company compensation structure.
