Wound Care Provider NP/PA

Ventura Wound Care is seeking a compassionate, motivated, and detail-oriented part time or full-time NP or PA to join our growing physician-led mobile wound care team serving Ventura County and Santa Barbara County. This role offers the opportunity to provide advanced wound care directly in patients’ homes, assisted living facilities, and care settings while working closely with home health agencies, caregivers, and our medical director in a highly collaborative environment.

Our practice specializes in comprehensive wound management for medically complex patients, including pressure injuries, diabetic ulcers, vascular wounds, surgical wounds, skin tears, and advanced wound therapies. We emphasize high-quality patient care, strong clinical documentation, teamwork, and meaningful relationships with patients and referral partners.

Responsibilities

• Perform comprehensive wound evaluations and follow-up visits in the field
• Develop and implement individualized treatment plans in collaboration with the supervising physician
• Perform bedside procedures including conservative sharp debridement, dressing applications, wound measurements, and advanced wound therapies
• Coordinate care closely with home health agencies, caregivers, specialists, and DME providers
• Complete timely and accurate EMR documentation
• Order wound care supplies, diagnostics, labs, imaging, and referrals as needed
• Educate patients and families regarding wound healing, nutrition, infection prevention, and overall care plans
• Maintain excellent communication with referral partners and interdisciplinary teams

Qualifications

• Active California NP or PA license in good standing
• DEA license preferred
• Wound care experience preferred, but willing to train the right candidate
• Experience in home health, geriatrics, internal medicine, vascular care, dermatology, surgery, hospice, or skilled nursing is a plus
• Spanish speaking strongly preferred
• Strong bedside manner and communication skills
• Comfortable working independently in a mobile setting
• Reliable transportation and willingness to drive throughout Ventura County and Santa Barbara County
