Position Summary:
The Field Supervisor is responsible for maintaining accurate records, conducting monthly individual visits, and following up on recommendations. The Supervisor will design and implement program components that uphold high expectations, promote social and personal growth, and cultivate a caring and engaging community.
Essential Responsibilities (including but not limited to):
- Conduct assessments to determine individuals’ goals.
- Supervise Community Habilitation (Com-Hab) staff and address sensitive issues as they arise.
- Communicate with parents/guardians regarding individuals’ progress.
- Track daily attendance.
- Maintain regular communication with families, Com-Hab staff, and the Director.
- Immediately notify the Director of any operational issues.
- Schedule individual visits one week in advance and post them in the weekly schedule.
- Obtain annual questionnaire surveys from individuals’ families.
- Compile program data to support individual progress and billing requirements (daily/weekly notes).
- Conduct monthly home visits to ensure staff performance aligns with established goals.
- Perform incident investigations (reportable and non-reportable).
- Complete monthly Community Habilitation documentation.
- Obtain required documentation from MSCs (psychological, medical, PPD, psycho-social, LoC, DDP, ISP, etc.).
- Support COVID documentation, Telehealth coverage, and EVV implementation.
- Train new and existing employees on writing goal sheets.
- Participate in scheduled meetings as assigned by the Director.
- Perform other related duties as assigned by the Director.
Qualifications:
- Bachelor’s degree in a Human Services–related field required.
- Must be able to communicate in Spanish
- Valid, unrestricted New York State Driver’s License required.
- Minimum of one (1) year experience working with individuals with developmental disabilities required.
- Demonstrated leadership experience, including staff supervision and program management.
- Experience documenting individual progress.
- Strong community inclusion skills.
- Excellent verbal and written communication skills.
- Must be fully vaccinated.
