MDS Coordinator / Staff Educator (RN) – Full-Time | Salary/Exempt

Overview

We are seeking an experienced MDS Coordinator / Staff Educator (RN) to lead the MDS/RAI process and support staff education initiatives. This role is essential in ensuring accurate assessments, timely care planning, regulatory compliance, and ongoing clinical development of nursing staff.

 

Key Responsibilities

• Coordinate and oversee the MDS/RAI process, including assessments, CAAs, and care plans
• Ensure timely and accurate completion of MDS assessments (admission, quarterly, annual, and change of condition)
• Lead and participate in Interdisciplinary Care Plan (ICP) meetings
• Monitor compliance with CMS, state, and federal regulations
• Maintain organized documentation systems for audits and survey readiness
• Collaborate with DON, Administrator, and clinical teams to improve outcomes
• Upload and manage MDS data in electronic systems (e.g., Abaqis or similar)
• Assist with QAPI initiatives and performance improvement projects
• Provide staff education and in-services related to MDS, care planning, infection control, and clinical standards
• Support direct resident care as needed and assist during staffing shortages
• Participate in surveys, audits, and regulatory inspections

Clinical & Documentation Duties

• Complete and review care plans to ensure appropriate goals and interventions
• Audit charts for accuracy, completeness, and compliance
• Document care plan meetings and resident updates
• Communicate changes in condition to physicians and family members
• Ensure proper coordination of discharge planning and resident transitions

Qualifications

• Active, unencumbered Registered Nurse (RN) license
• Minimum 2 years of MDS coordination experience OR 2 years of geriatric nursing experience
• Strong knowledge of RAI process, MDS, and care planning requirements
• Experience with electronic clinical systems and MDS submission platforms
• Excellent communication and organizational skills
• Ability to work collaboratively with interdisciplinary teams

Requirements

• Nursing degree from an accredited program
• Valid driver’s license and reliable transportation
• Ability to multitask, prioritize, and meet strict deadlines
• Must pass background check, drug screening, and license verification
