MDS Coordinator / Staff Educator (RN) – Full-Time | Salary/Exempt

Overview
We are seeking an experienced MDS Coordinator / Staff Educator (RN) to lead the MDS/RAI process and support staff education initiatives. This role is essential in ensuring accurate assessments, timely care planning, regulatory compliance, and ongoing clinical development of nursing staff.
Key Responsibilities
- Coordinate and oversee the MDS/RAI process, including assessments, CAAs, and care plans
- Ensure timely and accurate completion of MDS assessments (admission, quarterly, annual, and change of condition)
- Lead and participate in Interdisciplinary Care Plan (ICP) meetings
- Monitor compliance with CMS, state, and federal regulations
- Maintain organized documentation systems for audits and survey readiness
- Collaborate with DON, Administrator, and clinical teams to improve outcomes
- Upload and manage MDS data in electronic systems (e.g., Abaqis or similar)
- Assist with QAPI initiatives and performance improvement projects
- Provide staff education and in-services related to MDS, care planning, infection control, and clinical standards
- Support direct resident care as needed and assist during staffing shortages
- Participate in surveys, audits, and regulatory inspections
- Complete and review care plans to ensure appropriate goals and interventions
- Audit charts for accuracy, completeness, and compliance
- Document care plan meetings and resident updates
- Communicate changes in condition to physicians and family members
- Ensure proper coordination of discharge planning and resident transitions
- Active, unencumbered Registered Nurse (RN) license
- Minimum 2 years of MDS coordination experience OR 2 years of geriatric nursing experience
- Strong knowledge of RAI process, MDS, and care planning requirements
- Experience with electronic clinical systems and MDS submission platforms
- Excellent communication and organizational skills
- Ability to work collaboratively with interdisciplinary teams
- Nursing degree from an accredited program
- Valid driver’s license and reliable transportation
- Ability to multitask, prioritize, and meet strict deadlines
- Must pass background check, drug screening, and license verification
