Job Title: Home Health LPN
Location: Fairfax, VA 22031
Pay: $1,600/week
Schedule: Monday – Friday

Job Summary:
Seeking an experienced Home Health LPN to provide skilled nursing care to patients in their homes across the Fairfax and Prince William County area.

Responsibilities:

• Deliver direct patient care in a home setting

• Monitor patient conditions and report changes

• Assist with care plans and patient education

• Complete documentation in HCHB (Homecare Homebase)

Requirements:

• Active LPN license (VA)

• Minimum 1 year of home health experience

• HCHB experience required

• Valid driver’s license and reliable transportation
