Physical Therapist

We are seeking two Physical Therapists to join a Skilled Nursing Facility in Placerville, California. This role focuses on treating adult and geriatric patients in post-acute rehabilitation and long-term care settings.

This assignment offers a consistent schedule, guaranteed hours, and a structured clinical environment. You’ll work alongside a collaborative therapy team focused on safe mobility, functional improvement, and patient-centered care.

• Evaluate patients and establish individualized plans of care

• Provide skilled physical therapy interventions focused on mobility and function

• Monitor patient progress and update treatment plans as appropriate

• Collaborate with nursing and other therapy disciplines

• Complete documentation accurately and in a timely manner

• Active California Physical Therapist license

• Skilled nursing or post-acute experience preferred
