Description:The Licensed Practical Nurse provides nursing care to assigned patients under the supervision of a registered nurse in accordance with established nursing policies, procedures, and regulations. The LPN practices nursing consistent with the Illinois Nurse Practice Act, maintains records per facility guidelines, and is responsible for maintaining clinical competence.
Key Responsibilities
  • Provide patient care within the facility in alignment with nursing department standards.
  • Administer medications and treatments safely and efficiently. Perform assessments and interact therapeutically with patients, families, and staff.
  • Delegate tasks to ancillary personnel appropriately and keep Staff RN, House Supervisor, and CNO informed of patient conditions and concerns.
  • Document care delivery, assessments, and responses to treatment in the Electronic Health Record (EHR) per facility guidelines.
  • Maintain proficiency in tools, equipment, and nursing procedures required for the role.
  • Ensure a safe and therapeutic environment while utilizing resources efficiently.
  • Actively participate in care plan formulation and assess progress toward goals in the documentation process.
  • Assist physicians with treatments, procedures, and examinations.
  • Follow facility policy for IV therapy if certified and maintain IV therapy skills within the scope of practice.
  • Adhere to Compliance, Safety, and HIPAA policies of the facility.
  • Manage and protect sensitive patient information.
  • Demonstrate knowledge of CPSI systems.
Requirements:
  • Licensed in the State of Illinois and in good standing.
  • 1-year experience in an acute care setting preferred but not required.
  • IV certification preferred but not required.
  • CPR certification required.
  • Basic computer proficiency.

Physical Demands and Work Environments:

  • Regularly involves standing, walking, sitting, and using hands for fine dexterity.
  • Occasional lifting of up to 50 pounds; rare instances of lifting more than 100 pounds.
  • Exposure to wet/humid conditions, fumes, moving mechanical parts, and moderate noise levels typical of a clinical setting.
  • May encounter risks such as electrical shock, radiation, and toxic or caustic chemicals.
  • Close vision, distance vision, depth perception, and ability to adjust focus are necessary.
  • Ability to see computer screens and complete typing tasks both seated and standing.
  • Ability to hear alarms on equipment, patient calls, and instructions from physicians and staff.
  • Frequent use of hands for grasping and fine dexterity tasks.
  • Occasional repetitive use of foot controls and firm grasping.

Shift: Night Shift - additional shift differential added to hourly rate
