Responsibilities

Experience/Qualifications

  • Licensed Practical Nurse (LPN) license to practice in the respective state.

  • Provide nursing care under supervision while ensuring patient safety and comfort.

  • Accurate documentation of assessments, treatments, and outcomes in EMR systems.

  • Administer medications per protocol and monitor for adverse reactions.

  • Adhere to regulatory standards and workplace safety policies at all times.

  • Collaborate with physicians and care teams to develop and update care plans.

  • Track patient progress against defined targets and KPIs; report variances.

  • Utilize forecasting pipelines to anticipate patient care needs and staffing demands.

  • Engage in daily huddles, providing timely updates on patient status and workload.

  • Demonstrate eagerness to learn through active participation in training and development.
