Responsibilities

• Provide comprehensive occupational therapy assessments and develop individualized treatment plans.

• Deliver evidence-based OT interventions to improve functional independence.

• Document clinical notes, progress, and outcomes in electronic health records.

• Collaborate with multidisciplinary teams to coordinate patient care and discharge planning.

• Set and monitor performance targets using pipelines and metrics for territory outcomes.

• Manage a personal caseload while contributing to a high-visibility, results-driven environment.

• Engage in ongoing professional development and pursue advancement opportunities.

• Educate patients and families on therapy goals, home programs, and safety.

• Identify and report barriers to care, and propose process improvements.

• Participate in supervision and peer reviews to ensure clinical quality and compliance.

• Represent the OT service in meetings, planning sessions, and community outreach as needed.

Experience/Qualifications

• Licensed Occupational Therapist with active/state OT registration

• Evidence-based OT assessment and treatment planning

• Ability to set, monitor, and report on performance targets and metrics

• Experience documenting notes and outcomes in electronic health records

• Caseload management with time and priority management

• Strong collaboration with multidisciplinary teams for care coordination

• Commitment to ongoing professional development and career advancement

• On-site presence with willingness to engage in community outreach and planning

• Supervision, peer review participation, and adherence to clinical quality standards

• Patient and family education on goals, home programs, and safety
