Ultimate Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services within our skilled nursing and post-acute care setting. This position offers the opportunity to work closely with residents recovering from surgery, illness, injury, and mobility decline.

What You'll Do:

• Perform evaluations and develop individualized therapy programs
• Provide treatments focused on mobility training, balance improvement, strengthening, endurance, and functional movement
• Work collaboratively with rehab staff and interdisciplinary care teams
• Educate residents and families on exercises, safety, and mobility techniques
• Maintain timely and compliant documentation

Qualifications:
