Responsibilities

• Provide patient care under supervision, documenting observations and outcomes accurately.

• Administer medications and treatments following protocols and safety guidelines.

• Maintain accurate and timely patient records for compliance and auditing.

• Identify and escalate clinical concerns promptly to supervising clinician.

• Ensure compliance with healthcare regulations, privacy laws, and facility policies.

• Assist with patient assessments and vital signs to support care planning.

• Collaborate with a multidisciplinary team to achieve patient care goals.

• Participate in ongoing professional development and skill enhancement.

• Demonstrate reliability in a results-driven environment with transparent performance metrics.

• Support goal-oriented targets through efficient workflow and time management.

• Engage in quality improvement initiatives to optimize patient care processes.

• Provide compassionate patient and family education on treatments and care plans.

Experience/Qualifications

• Active LPN license in the applicable state or region.

• Prior patient care experience in acute or ambulatory settings.

• Ability to document observations and outcomes accurately and promptly.

• Medication administration competency following safety protocols.

• Strong understanding of privacy laws and healthcare compliance standards.

• Proven ability to work under supervision within a multidisciplinary team.

• Demonstrated reliability in a metrics-driven, target-focused environment.

• Experience with patient assessment, vital signs, and care planning.

• Commitment to ongoing professional development and skill enhancement.

• Excellent communication skills for patient and family education.
