PHYSICAL THERAPIST (PT) – PERMANENT OR TRAVEL (SNF)
Location: CA 95667
Start Date: ASAP
Schedule: Permanent or Travel
Compensation:
$58 – $70 per hour
Evaluate and treat SNF residents
Deliver skilled therapy and document care
Ensure Medicare/PDPM compliance
Collaborate with interdisciplinary team
Maintain productivity and quality standards
Active Physical Therapist (PT) license
SNF or post-acute rehab experience required
Knowledge of Medicare/PDPM and clinical documentation standards
Strong communication and teamwork skills
