Physical Therapist (PT) – Travel Contract

Pay: $2,600 per week
Contract Length: 13 weeks (extension possible)
Schedule: Day shifts | 36–40 hours per week

We’re looking for a motivated and compassionate Physical Therapist (PT) to join a Skilled Nursing Facility/Acute in Oak Valley, CA. Responsibilities
Assess and treat patients in skilled nursing and acute settings, following PT plans of care.
Develop individualized treatment plans with measurable outcomes and timelines.
Collaborate with multidisciplinary teams to coordinate patient care and discharge planning.
Meet or exceed productivity targets and contribute to pipeline-based planning.
Document patient progress and maintain accurate records for team visibility.
Perform re-evaluations and adjust interventions based on data and goals.
Demonstrate and model evidence-based practice and continuous learning.
Maintain compliance with regulatory standards and facility policies.
Educate patients and caregivers on home exercise programs and safety.
Participate in on-site quality improvement initiatives and outcomes reporting.
Identify opportunities for professional development and pursue relevant certifications.
Experience/Qualifications
Licensed Physical Therapist with active California state licensure in SNF/acute settings.
Proven experience in skilled nursing and acute care therapy assessment and treatment.
Demonstrated ability to develop and document measurable treatment plans with timelines.
Compliance with regulatory standards and facility policies; strong ethics.
Strong collaboration with multidisciplinary teams and discharge planning.
Productivity-focused with ability to meet or exceed targets in a pipeline-driven environment.
Accurate documentation and data-driven re-evaluation for intervention adjustments.
Evidence-based practice emphasis with commitment to continuous learning.
Patient and caregiver education on home exercise programs and safety.
Participation in on-site quality improvement initiatives and outcomes reporting.
Pursuit of relevant certifications and professional development opportunities.
