Responsibilities
Provide licensed practical nursing care to patients following care plans.
Accurately document patient assessments, treatments, and outcomes in EHR systems.
Monitor patient vital signs and report changes to the supervising nurse promptly.
Administer medications and treatments as prescribed, observing safety protocols.
Collaborate with a multidisciplinary team to coordinate patient care plans.
Meet performance targets through efficient patient throughput and quality care.
Maintain compliance with regulatory standards and quality assurance protocols.
Proactively identify improvement opportunities and contribute to process optimizations.
Educate patients and families about care plans, procedures, and post-discharge needs.
Demonstrate accountability by delivering own results within a team-driven environment.
Participate in on-site training programs to support professional growth and role expansion.
Experience/Qualifications
Current LPN license in the applicable state with active registration
Accurate medication administration following prescribed protocols
Proven ability to document assessments and interventions in an electronic health record
Vital signs monitoring and prompt escalation to supervising nurse
Compliance with regulatory standards and quality assurance protocols
Collaborative, multidisciplinary teamwork to coordinate care plans
Commitment to efficiency and achieving throughput targets
Proactive identification of process improvements and participation in optimization
Patient and family education on care plans, procedures, and post-discharge needs
On-site training participation and ongoing professional development
Strong accountability for individual results within a team-driven environment
Experience with on-site onboarding and adherence to shift schedules
Ability to thrive in a growth-oriented, merit-based environment
