We are seeking a skilled and compassionate Physical Therapist to join a well-established skilled nursing and rehabilitation center in Lincoln, CA. This facility provides post-acute rehab, long-term care, and restorative programs for adults and seniors recovering from illness, surgery, or functional decline.
In this role, you will conduct evaluations, build individualized treatment plans, and provide hands-on therapeutic interventions focused on improving mobility, reducing fall risk, enhancing strength, and supporting safe functional independence. This is a collaborative interdisciplinary environment with a strong emphasis on communication, quality outcomes, and resident-centered care.
This full-time assignment offers a consistent caseload and a supportive rehab team committed to helping patients achieve meaningful progress.

Responsibilities
  • Conduct patient assessments to develop personalized treatment plans.

  • Implement therapeutic exercises and functional mobility programs to improve patient strength and independence.

  • Set and achieve personal performance and productivity targets.

  • Monitor patient progress and modify care plans as needed.

  • Participate in ongoing professional development and clinical training.

  • Educate patients and families on treatment plans, mobility strategies, and home exercises.

  • Maintain detailed, accurate, and timely documentation.

  • Utilize evidence-based treatment approaches to support optimal outcomes.

  • Collaborate with nursing, physicians, and other therapists to ensure coordinated care.

  • Contribute to quality-improvement initiatives and patient-experience enhancements.

Experience / Qualifications
  • Valid state license to practice as a Physical Therapist.

  • Experience in SNF or post-acute care preferred, but not required.

  • Strong evaluation, clinical reasoning, and treatment-planning skills.

  • Ability to communicate effectively with patients, families, and interdisciplinary staff.

  • Proficiency with documentation and EMR systems.
