Responsibilities

• Provide outpatient dietary assessments and develop individualized nutrition plans.

• Maintain regulatory compliance and ensure quality standards in outpatient services.

• Monitor patient progress and adjust plans to meet clinical targets.

• Educate patients on nutrition, lifestyle changes, and dietary compliance.

• Meet defined performance targets and contribute to pipeline-based forecasting.

• Collaborate with physicians and care teams to optimize treatment plans.

• Document all consultations and outcomes in electronic health records.

• Utilize data and metrics to identify opportunities for improved patient outcomes.

• Participate in continuous professional development and credential maintenance.

• Assist in care coordination for referral networks and follow-up scheduling.

• Support team-based culture by sharing best practices and mentoring junior staff.

• Participate in patient outreach and education events to raise service visibility.

Experience/Qualifications

• Registered Dietitian (RD) with state licensure in Iowa

• Outpatient nutrition assessment and personalized plan development

• Documentation in electronic health records with HIPAA compliance

• Monitoring progress and adjusting plans to meet clinical targets

• Collaboration with physicians and care teams for integrated care

• Knowledge of outpatient regulatory standards and quality metrics

• Patient education on nutrition, lifestyle changes, and adherence

• Data-driven approach to identify opportunities for better outcomes

• Participation in care coordination for referrals and follow-up scheduling

• Active participation in professional development and credential maintenance
