LPN

Position: LPN (Contract-to-Perm)

• Location: NY 14527

• Contract Length: 05/12/2026 – 08/15/2026 (~13 weeks)

• Pay: $1,326 – $1,495/week

• Schedule: Part-time, 2–3x8-hour variable Day/Evening shifts (e.g., 7:00 AM – 3:00 PM or 3:00 PM – 11:00 PM)

• Hours: ~24 hours/week

• Additional Requirements: Every other weekend (EOW) and holidays required

• Provide direct/indirect care to adult & geriatric residents under RN supervision

• Monitor/report patient status and assist with care planning

• Support ADLs, safety, nutrition, and restorative care per plan of care

• Collaborate with interdisciplinary team and maintain accurate documentation

• Ensure compliance with safety, infection control, and care standards

• Active NY LPN License (required)

• BLS certification (AHA or RQI required)

• Minimum 1 year LPN experience (LTC preferred)

• High School Diploma or GED required

• Experience with adult and geriatric populations

• Strong communication and teamwork skills

• Ability to work variable shifts, weekends, and holidays

• Comfortable with potential exposure to blood/body fluids
