Physical Therapist

Contract: 13 weeks
Hours: 36 hours/week (guaranteed)

Physical Therapist needed for a Skilled Nursing Facility in Auburn, California providing rehabilitative care to post-acute and long-term care patients. This role focuses on restoring mobility, strength, and functional independence in a structured SNF setting.

• Evaluate patients and develop individualized plans of care

• Provide skilled physical therapy interventions

• Monitor patient progress and modify treatment plans as needed

• Collaborate with interdisciplinary care teams

• Complete timely and accurate documentation per facility guidelines

• Active California Physical Therapist license

• Skilled Nursing Facility or post-acute care experience preferred
