LPN

Position: LPN (Contract-to-Perm)

• Location: NY 13165

• Contract Length: 05/12/2026 – 08/15/2026 (~13 weeks)

• Pay: $1,269 – $1,432/week

• Schedule: 8-hour shifts (12-hour clock format)

• Orientation Requirement: Must be able to start full-shift orientation on a Tuesday or Thursday

• Minimum Shift Requirement: 24 hours (3 shifts) within a 4-week period; at least 2 weekend shifts required

• Additional Requirements: Every other weekend (EOW) and holidays required

• Provide direct/indirect nursing care to adult & geriatric residents under RN supervision

• Monitor patient status; collect/report clinical & psychosocial data

• Support, implement, and update individualized care plans

• Collaborate with interdisciplinary team, residents, and families

• Maintain competency, ensure safety, and follow all clinical protocols

• Assist with staff orientation/mentorship and uphold quality care standards

• Active NY LPN License (required)

• BLS certification (AHA or RQI required)

• Minimum 1 year LPN experience, preferably in LTC setting

• Experience with adult and geriatric populations

• Ability to meet scheduling requirements (weekends, holidays, orientation days)

• Comfortable with potential exposure to blood/body fluids
