PHYSICAL THERAPIST (PT) – PERMANENT OR TRAVEL (SNF)
Location: CA 95209
Start Date: ASAP
Schedule: Permanent or Travel
Compensation:
$60 – $65 per hour
Evaluate and treat SNF residents
Provide and document skilled therapy services
Ensure Medicare/PDPM compliance
Collaborate with interdisciplinary care team
Maintain productivity and quality standards
Active Physical Therapist (PT) license
SNF or post-acute rehab experience required
Knowledge of Medicare/PDPM guidelines and documentation
Strong communication and teamwork skills
