LPN

Location: NY 13165
Pay: $1,258 – $1,420/week
Schedule: Part-Time Evenings, 3 shifts per week (rotational: 2 shifts wk1, 3 shifts wk2)
Additional: Every other weekend & holiday coverage required

• Provide direct and indirect skilled nursing care to adult and geriatric residents

• Collect and communicate resident data to RN and interdisciplinary team

• Assist in development, evaluation, and revision of resident care plans

• Collaborate with residents, families, and healthcare team to ensure safe, age-appropriate care

• Maintain nursing knowledge and competency; mentor/orient staff as needed

• Active LPN license (NY) – Required

• BLS (AHA or RQI) – Required

• Minimum 1 year LPN experience in a long-term care (LTC) setting

• Knowledge of adult/geriatric care principles

• Occupational exposure to blood/body fluids may occur

• Must follow defined policies and procedures under RN supervision
