Ultimate Therapy is hiring a Physical Therapist (PT) to join our rehabilitation team within a skilled nursing facility. This role focuses on helping residents improve mobility, strength, balance, and overall functional independence through individualized therapy services.

Responsibilities:

• Complete physical therapy evaluations and establish treatment plans based on resident needs
• Provide therapy interventions focused on gait training, transfers, strengthening, balance, endurance, and pain management
• Collaborate with PTA staff, nursing, physicians, and interdisciplinary team members to support resident care goals
• Monitor resident progress and adjust treatment approaches as appropriate
• Maintain accurate documentation and treatment records

Qualifications:

• Active PT license required
