SUBMISSION REQUIREMENTS

All required documentation must be submitted no later than 5 business days prior to start
Provider questionnaire required at time of submission
NPDB self-query required at time of presentation, within 6 months
Full disclosure on all malpractice required
Do not send open-ended presentations
If the role includes specific coverage dates, your submission must clearly state exact dates the provider can work
No follow-up required—if we have to ask for availability, the submission will be rejected
Submissions without rates will be rejected immediately
Required Submission Packet: CV, Coversheet, Questionnaire, NPDB (pulled within the last 6 months)

Duties & Responsibilities

Provide sporadic solo night coverage
Perform regional block (nerve block) anesthesia for high volume of ortho patients
Provide OB anesthesia coverage
Deliver high quality, patient centered care across a broad range of surgical and procedural areas
Cover a 1:3 CRNA/room ratio
