About this Opportunity
The Staff Pad has partnered with Blue Ridge Medical Center, located in Blue Ridge, GA to hire highly motivated Swing Bed Coordinator to lead and grow their Swing Bed Program, support care transitions, patient flow, and program growth within a critical access hospital setting.
This is a key leadership role for an RN, LPN, or Case Manager with strong clinical expertise and hands-on Swing Bed experience who thrives in a collaborative, patient-centered environment. The Swing Bed Coordinator is responsible for overseeing and taking operational ownership of all aspects of the hospital’s Swing Bed Services, including daily census management, CMS compliance, documentation accuracy, referral management, interdisciplinary coordination, and program financial performance to ensure high-quality patient outcomes and operational success.
Your leadership will directly impact patient outcomes, program growth, regulatory success, and overall hospital performance.

What You'll Do
The Swing Bed Coordinator serves as the clinical and operational leader for admissions, patient care progression, and discharge planning, ensuring each Swing Bed stay meets Medicare skilled care requirements, is supported by accurate and compliant documentation, and advances efficiently toward a safe and timely discharge.
Lead Swing Bed Operations
- Oversee daily Swing Bed census, bed availability, and admission prioritization
- Evaluate and approve admissions based on CMS skilled care criteria
- Monitor patient progression, length of stay, and discharge planning status
- Serve as the primary resource and point of coordination for all Swing Bed activity
Ensure Compliance & Documentation Excellence
- Maintain compliance with CMS Swing Bed Conditions of Participation
- Review skilled nursing, therapy, and provider documentation for accuracy and completeness
- Proactively identify and resolve documentation deficiencies in real time
- Maintain survey-ready and regulatory-compliant documentation standards
Manage Referrals & Program Growth
- Coordinate referral intake, admissions, and acceptance decisions
- Monitor and track referrals, bed capacity, and admission conversion metrics
- Build and maintain strong referral relationships with internal and external partners
- Support program growth initiatives and community outreach efforts
Collaborate Across Clinical Teams
- Partner closely with nursing, therapy, providers, and case management teams
- Ensure timely therapy evaluations and coordinated care planning
- Facilitate interdisciplinary communication to support efficient care progression and discharge
- Promote best practices in skilled documentation and patient care coordination
Support Financial & Operational Performance
- Monitor census patterns, average length of stay, and throughput metrics
- Assist with accurate charge capture and billing processes
- Participate in denial prevention strategies and documentation audits
- Provide program performance updates and metrics to leadership
Contribute to Program Development
- Maintain and improve Swing Bed policies, workflows, and admission criteria
- Educate staff on regulatory requirements and documentation expectations
- Support diagnosis expansion and pathway development
What We Are Looking For
- Minimum of 3 years of direct Swing Bed experience
- Strong understanding of Medicare skilled nursing criteria and CMS Swing Bed Conditions of Participation
- Background in acute care, med-surg, case management, or utilization review preferred
- Ability to work independently while collaborating effectively across departments
Education, Qualifications, Certifications and Licenses
- Active, unrestricted RN or LPN license
- Case Management experience required
